The correction of penis curvature
Outpatient surgery and highly developed suture material that is inert and non allergic and the development of microsurgery enabled this operation to be performed safely on the penis. The simultaneous use of penis prosthesis is sometimes important and complementary to the procedure.

Penile curvature is caused by the development of fibrosis on the contra lateral side of the curvature of the penis at the area of the facial investment of the erectile tissue of the penis, namely the corpus cavernosum. The reason for that may be immunological, traumatic or due to any type of injection and in particular if the latter has been used for a long time.

a = penis curvature before surgery

a = reduction of the scar on the erectile

a = penis straightened

Who is qualified for the operation?

Patients who have a penis curvature of any degree due to injuries of the penis, induratio penis plastica (IPP or pyroni `s disease) or patients who inject themselves locally for the treatment of impotence if this results in local fibrosis.

How does the operation work?

The patient lies on the back. The penis is incised at the side of foreskin. The fibrosed area is made ready for surgery. The erectile tissue is injected with pharmacological agent to prove erection and localise the diseased area. The nerves and vessels (arteries and veins) of the penis are made ready for surgery though a fine microsurgical dissection to protect them from injury. Doubled sutures are put on the contra lateral side of the curvature to straighten the penis. In advanced cases the fibrosed area is excised and closed or even replaced and covered by a piece of pre treated pericardium. The neurovascular bundle is replaced on the dorsum of the penis. The incision is closed.

In some complicated cases where the septum of the penis is fibrosed and hardened through deposition of calcium plaques, it is necessary to insert penis prosthesis and manipulate the hard calcified plate accordingly.

Healing:

It requires ten days to achieve complete healing. The hospital stay is maximum two days. This procedure is usually done on an out patient basis.

Frequently Asked Questions (FAQ):

  1. What is pyroni`s disease?
    --
It is the development of scar tissue in the tissue investing the erectile bodies (tunica albugenia) and the septum in between. This is due to an immunological disease.
    --
  2. Does it affect the intercourse?
    --
Yes, it produces plaques leading to the curvature of the penis whether to look up or down. The degree of the angle of curvature may return the penetration difficult.
    --
  3. Is it progressive?
    --
Yes, it may progress to involve a large area of the penis and may calcify into a bone like structure.
    --
  4. Is there medical treatment?
    --
This is only possible in the very early stage (acute stage). The oral medication of tamoxifen or the local injection of cortisone, interferon may stop the progression in this stage.
    --
  5. What is the surgical treatment?
    --
Duplication of the tunica albugenia with sutures and even removal of the plaque. In advanced cases where there is calcification the only treatment may be the implantation of penis prosthesis.
    --
  6. Does it have effects on potency?
    --
Yes, it may lead to the development of venous insufficiency or makes cohabitation impossible. The operation itself may lead to impotance in case of removal of the plaque. The other operative risks like infection, bleeding, collection of coagulated blood or serum under the skin are minimal.
