Impotence results from the disease of the veins, nerves, arteries or tissues of the penis. The failure of the valves of the veins of the penis to close during erection leads to the return of the blood trapped inside the penis to the body. This leads to rapid softness of the penis or to complete abscence of erection.  This may be accompanied with an insufficiency of the artery or disease of the tissues. The best treatment in such cases is the implantation of penis prosthesis. This should be done after trying medical treatment which may be of benefit in mild cases.

Another option of treatment is to ligate the veins of the penis along the shaft. It is an operation of limited success but one may resort to it if it is wished not to implant a penile prosthesis at leat for some time. In some cases it is observed that Viagra may help after the operation if the case is mild.

Who qualifies for the operation?

Patients who have a mild venous leakage as confirmed by pharmacodynamic tests and radiological examination in particular if they want to postupon the implant ation of penile prosthesis for sometime.

How does the operation work?

The patient lies on his back. Incision is done either at the base of the penis or behind the sulcus coronarius to expose the shaft of the penis and the veins. All veins are carefuly selected and disected. They are removed without affecting the adjacent structures. The shaft is covered by the skin and the incieion is closeed and dressed.

    No, this is impossible.

  2. Is there any predisposition for the disease?

    This is not known but diabetes mellitus and weakness of the connective tissue of the body in some patients may be a cause.

  3. What is the success rate of the operation?

    It is 25% after 2 years of observation. This means only 25% of the operated patients retain their potency after the laps of two years after the operation.

  4. Does this mean that the ultimate treatment of venous leak is the implantation of penile prosthesis?

    In most cases yes. Overcoming venous leakage through the ligation of the veins is of limited success. The recurrance rate is high. It was observed that in few cases, also in my practice, the intake of Viagra postoperatively have helped patients to have a better erection.

  5. What are the other complications of the surgery?

    The surgery of the penis may be complicated by infection in the area, injury to the nerves or the artery or even my cause impotence or in extreemly rare cases loss of the shaft of the penis.
