Anesthesia
Local anesthesia

Regional anaesthesia or local anaesthesia is the method of choice to kill pain in a limited superficial area of operation. It is prefered in outpatient surgery and in genital organs. The technique of this type of pan killing is called field block as it is injected under the skin near the course of superficial nerves to stop pain conduction through them.

There are many drugs used for producing local anaesthesia. The most important group of it is the cain group like lidocain, scandicain. xylocain and others. They may be mixed with a drug to produce narrowing of the blood vessels of the area to delay the wash out of the anaesthesia and hence to prolonge its action. These mixtures are used in certain areas but not the tapering or prepheral end parts of the body. Inspite of being local, it is still considered an anaesthesia which affects not only pain but behaviour as well. That is why it is still advised to appear fasting for the operation even if it is done under local anaesthesia. You can not share in public trafic or take any decision at the day of the local anaesthesia.

Who qualifies for the local anaesthesia?

All cooperative adults undergoing minor and some superficial operationsof a time not exceeding two hours regardless of being outpatient or inpatient operation.

How does the anaesthesia work?

It is injected under the skin in long tracks under complete sterile condition. The injection is undertaken with a safety margin arround the field of the operation to insure complete killing of the pain during the operation and for sometime afterwards. Some other protective injections may be given prior to the injection of certain local anaesthetics to prevent eventual side effects on the heart.

Frequentely asked questions (FAQ):
--

  1. Is local anaesthesia suitable for every operation?
    --
No, It is suitable to short lasting operations only.
    --
  2. Can I go home immediately after an operation done under local anaesthesia?
    --
No, You should stay for sometime to recover. Local anaesthesia affects the endings of the nerves and also your nervous system. You should keep in mind that even under local anaesthesia you should be under care of some other person during your recovery and for the whole day.
    --
  3. What are the side effects of local anasthesia?
    --
This depends on the type of solution used. They are hawever safe. Local narcotica have rarely side effects if any if used as indicated. The major side effects which may happen are allergic reaction, sudden high increase of temperature, slowing of the heart rate, heart insufficiency, cramps, vomiting.
    --
  4. Can I take some pain killing tablets to help the local anaesthesia?
    --
No, This is not recommended at all and may be dangerous.
    --
  5. Can local anaesthesia be given to children?
    --
No, It needs some cooperation and calmness from the patient which is difficult to be provided by children.

General anesthesia
The usual general anaesthesia in cases of ambulant surgery is the laryngeal mask anaesthesia. It is safe and permits a long time operation up to 4 hours. It is easy to administer and to be tolerated by the patient. It is always preceeded by light sedation and induction where a pain killing and sleep initiating drug is injected into the vein. The anaesthesia is then given as a gas through a tube connected to a mask covering the larynx.

As a general anaesthetic it affects the general nervous system both the motional, intlectual and emotional functions. The patient should fasten for at least 6 hours before the anaesthesia. He is not allowed to take any activity of his own what so ever for 24 hours after recovery from the anaesthesia. That is why it is necessary to have a company after the surgery.

Who qualifies for the mask anaesthesia?

All patients undergoing an operation for longer than an hour or those who will be operated in deep parts of the body regardless of the length of the operation. It is also recommended to anxious patients. It is the anaesthesia of choice for children regardless of the procedure done.

How does the anaesthesia work?

The patient will take a sedative to calm him an hour before taking him to the theater. He will have a vein catheter which permits the injection medicatins and the infusion of solutions. A sleep initiating drug and a pain killer will be injected into the vein. The mouth is then opend and the anaesthesia mask is inserted to cover the larynx. The edges of the mask are inflated with air to make it tight on the opening of the larynx. This optimaly helps the administration of the anaesthesia and prevents the possibility of back going of  any food into the lungs in case of accidental vomiting. The tube of the mask is then connected to the anaesthesia apparatus which is adjusted to the suitable dosis of anaesthesia.

Frequently asked questions (FAQ):
--
  1. Is it possible to better operate under local anaesthesia for a rapid recovery?
    --
No, the length of the oeration indicates the type of anaesthesia. The recovery from mask anaesthesia is usually smooth and the patient is able to leave within a maximum of two hours after the operation.
    --
  2. What are the complications of mask anaesthesia?
    --
There may be food regurge into the larynx (avoided by fasting), post anaesthesia headache, Numbness, Paraesthesia, throat pain, temprary change of voice for a shor time and pain during swallowing for few days after the anaesthesia.
    --
  3. What are the advantages of mask anaesthesia?
    --
It permits for a long operation with acompanying muscle relaxation, the recovery is smooth and there is a post operative amnesia 8the patient forgets what happened to him before and immediately after the anaesthesia.
    --
  4. Is mask anaesthesia the optimal type of anaesthesia for children?
    --
It is by nature that the children are anxious and not cooperative. That s why it is recommended to operate them under general anethesia to relax them and to let them avoid the psychic trauma of the anaesthesia and operation.
    --
  5. When can I perform my activity after the anaesthesia?
    --
It is highly recommended to be back to your activity at least 24 hours after the anaesthesia. General anaestesia affects your psych and mental activity. You are not able to judje in the immediate post anaesthesia period.
    --
  6. When can I eat after the anaesthesia?
    --
